The prothrombin time (PT) analysis is employed to evaluate the efficiency of the extrinsic coagulation pathway and to detect inherited or acquired abnormalities in specific coagulation factors (such as factors II, V, VII, X, and fibrinogen). This test, available at the private laboratory IntroLab in Belgrade, can also be used to assess the synthetic abilities of the liver, specifically its capacity to produce proteins. Prothrombin time is also valuable for monitoring therapy with coumarins, a type of oral anticoagulants.

When is the prothrombin time analysis utilized?

Prothrombin time serves various purposes within the medical context, including:

  • Diagnosis of coagulation disorders: Prothrombin time, in conjunction with activated partial thromboplastin time (aPTT), aids in the diagnosis of blood clotting disorders. PT analysis identifies alterations in the external coagulation pathway, while aPTT identifies changes in the internal coagulation pathway. These tests can uncover deficiencies in specific coagulation factors (e.g., factors II, V, VII, X) or the presence of coagulation inhibitors, such as antiphospholipid syndrome. Modifications in prothrombin time can serve as indicators of unexplained bleeding or thrombosis, nosebleeds, bruising, or gum bleeding.
  • Preoperative patient preparation: Prothrombin time is often determined as part of patient preparation for surgery, particularly in cases of surgeries that may result in significant blood loss. This analysis assists medical professionals in evaluating a patient’s coagulation capabilities and taking appropriate measures to minimize the risk of bleeding during surgery.
  • Assessment of liver damage: Prothrombin time can provide insights into liver functionality. In instances of severe liver damage or conditions such as cirrhosis or hepatitis, alterations in prothrombin time can indicate the stage of liver damage. This information proves beneficial in assessing a patient’s condition and planning appropriate therapy.
  • Monitoring oral anticoagulant therapy: Prothrombin time, expressed as the international normalized ratio (INR), constitutes a fundamental parameter for monitoring the effectiveness of oral anticoagulant therapy. Oral anticoagulants, such as warfarin, inhibit the function of vitamin K-dependent coagulation factors, thereby preventing excessive blood clot formation in patients susceptible to thrombosis. INR is employed to maintain a balance between the risk of bleeding and clot formation, with therapy adjusted according to defined therapeutic ranges and doses. This analysis is especially critical for patients with heart conditions like atrial fibrillation or those with artificial heart valves.

Interpreting prothrombin time blood test results

In healthy individuals, prothrombin time typically falls within the reference range, indicating normal blood coagulation function. However, in patients undergoing anticoagulant therapy, reduced prothrombin time values expressed as the international normalized ratio (INR) may suggest an increased risk of blood clot formation (values less than 2). It’s important to note that INR values at the beginning of therapy are often unreliable, so more frequent monitoring is required to achieve stabilization. Subsequently, periodic INR monitoring is usually sufficient to assess the effectiveness of anticoagulant therapy.

Prolonged prothrombin time indicates slower clot formation and is often observed in:

  • Patients on oral anticoagulant therapy (OAT)
  • Severe liver damage
  • Vitamin K deficiency
  • Inherited or acquired coagulation factor deficiencies
  • Cases of massive transfusions
