Definition

Acid phosphatase is an enzyme that, in combination with its various isoforms, can be found in the liver, spleen, milk, red blood cells, platelets, bone marrow, and in men, the prostate. Half of the total activity of acid phosphatases in male serum comes from the prostate. The remainder originates from the liver, disintegrated platelets, or red blood cells. In women, on the other hand, serum activity mostly comes from the liver, erythrocytes, and platelets.

Prostatic acid phosphatase, as an isoenzyme of acid phosphatase, plays a crucial role in diagnosing malignant prostate diseases. In the case of prostate cancer, the concentration of this enzyme in the serum significantly increases. However, even in benign situations, such as inflammation and hypertrophy of the prostate, there can be a moderate increase in acid phosphatase levels, so the question of whether a blood analysis shows a tumor or carcinoma cannot be affirmatively answered if it is the only diagnostic tool.

Conditions like benign hypertrophy, prostatitis, and prostate infarction can result in increased activity of prostatic acid phosphatase. Analysis of this enzyme in serum can be useful in monitoring the remission or recurrence of prostate cancer. It can also be used to assess the effectiveness of various therapeutic strategies. After medical interventions on the prostate, such as surgeries, catheterizations, or radiotherapy, elevated levels of this enzyme can persist for a period of 1 to 2 days.

When is prostatic acid phosphatase analysis indicated?

The test for prostatic acid phosphatase is used when the doctor suspects that the patient may have advancing and metastatic prostate cancer. However, due to its non-specificity, this test has increasingly been replaced by more sensitive tests in recent times. This includes biochemical analyses for prostate-specific antigen (PSA) and its free form (FPSA).
