CA 15-3 (breast) is a tumor marker whose primary function is to help monitor the disease in patients who have already been diagnosed with breast cancer. This analysis is specific to this type of cancer, but it is not a perfect method for diagnosis or screening due to its limited sensitivity and specificity. What is CA 15-3 (breast)?

The protein known as cancer antigen 15-3 (CA 15-3) is produced in normal breast cells. Its concentration in the blood may be higher in women diagnosed with breast cancer. Therefore, it is often used to monitor treatment therapy and to detect the recurrence of the disease in individuals in remission. In addition to CA 15-3, additional tests and clinical examinations, including analyses for estrogen and progesterone receptors, Her2/neu, and genetic expression tests, are used for a comprehensive assessment of the cancer condition.

The concentration of CA 15-3 in the blood can be higher not only in breast cancer but also in other conditions. These conditions include acute hepatitis, liver cirrhosis, endometriosis, inflammatory diseases of the pelvic region, and various types of other cancers such as lung, colon, and pancreatic cancer. Additionally, the level of this protein is somewhat higher during pregnancy. However, it is important to note that although the values of CA 15-3 in these conditions are higher than normal, they are still lower than in individuals suffering from breast cancer.

When is CA 15-3 (breast) analysis indicated?

Testing for CA 15-3 is usually conducted along with other examinations at the initial diagnosis of advanced breast cancer. The goal is to determine the characteristics of the tumor and treatment options accurately. If the initial results show an elevated concentration, further periodic testing is conducted to monitor the effectiveness of the therapy and possible recurrence of the disease. However, it is worth noting that CA 15-3 is rarely tested for early detection of breast cancer before metastasis occurs, as the levels of this marker are usually not elevated in the early stages of the cancer.

Interpreting the results

Decreased values have no clinical significance.

The normal or reference values for CA 15-3 are less than 30 U/mL. However, slight to moderate increases can occur in various types of cancer, including lung, pancreas, ovarian, prostate, and colon cancers. Elevated CA 15-3 can also be detected in benign breast changes, as well as in individuals with cirrhosis or hepatitis, and even in a certain number of healthy individuals. In non-cancerous diseases, the levels of this protein often remain stable over time.

The concentration of CA 15-3 typically rises with tumor progression—the higher its level, the more advanced the breast cancer and the larger the tumor. In metastatic carcinoma, the highest values are usually observed when cancer affects the liver or bones. On the other hand, if the concentration of CA 15-3 increases over time, it may indicate that the therapy is not yielding the desired results or that the disease is recurring.

Note

The test for CA 15-3 itself does not possess adequate specificity or sensitivity to be used as a preventative test for detecting cancer, as many non-cancer-related conditions can result in elevated levels of this tumor marker. Additionally, patients who have been previously exposed to mouse antigens—through medical treatments, diagnostic procedures, or even from their environment—can develop antibodies in their bloodstream. These antibodies can interfere with the test results, creating the potential for obtaining inaccurate results.
